Understanding levels of care

Not sure which kind of care is right? Here is a plain-language overview of the four most common options in California. A Sérénity coordinator can help you match care needs to the right setting — at no cost.

Assisted Living Communities

Larger communities, often with dozens of residents or more, offering private apartments, meals, activities, and help with daily tasks such as bathing, dressing, and taking medications. In California they are licensed as Residential Care Facilities for the Elderly (RCFEs).

Often a good fit for: seniors who are mostly independent and enjoy a social, amenity-rich setting.

6-Bed Boutique RCFEs (Board and Care Homes)

Licensed residential homes in ordinary neighborhoods that care for six or fewer residents. The small size means more caregiver time per resident, home-cooked meals, and staff who know each person well. Many are approved to care for residents on hospice, residents who cannot walk unassisted, or residents living with dementia.

Often a good fit for: seniors who need more personal attention in a home-like setting.

Memory Care

Specialized care for Alzheimer’s disease and other dementias, with trained staff, structured daily routines, and secure surroundings — such as delayed-egress doors — that help prevent unsafe wandering. In California, every RCFE that cares for residents with dementia must meet additional Title 22 dementia-care rules, and homes that advertise memory care must meet further program requirements.

Often a good fit for: seniors living with dementia who need supervision and gentle redirection.

Skilled Nursing Facilities (SNFs)

Medical facilities with licensed nurses on duty around the clock, licensed by the California Department of Public Health. They provide rehabilitation after a hospital stay and ongoing skilled medical care that residential care homes cannot provide.

Often a good fit for: seniors who need daily skilled nursing or short-term rehabilitation.

Is Sérénity’s placement service really free?

Yes. Placement and transitional advocacy are 100% free to seniors and families, always. You will never receive a bill from Sérénity for helping you find care. Sérénity is paid by the licensed care homes in our partner network, never by families. You pay only the care home you choose, under that home’s own admission agreement.

How quickly can my loved one move in?

When a move is urgent, we coordinate placement so move-in can happen within 24 hours of clinical clearance — that is, once the hospital or physician confirms your loved one is ready for residential care. Timing also depends on bed availability and the admission paperwork the care home must complete, and we keep you informed at every step.

What is a hospice waiver?

In California, a residential care home for the elderly (RCFE) needs approval from the California Department of Social Services — usually called a hospice waiver — before it can accept or keep a resident who is receiving hospice care. A licensed, Medicare-certified hospice agency provides the medical and comfort care, while the home’s staff continue daily care. We only match residents on hospice with homes that hold this approval.

How do care home staff help with medications?

Care home staff don’t “administer” medications. Under California Title 22 rules they assist residents with self-administration of medications as prescribed by the physician — for example, when a resident has a tremor or failing eyesight — and they may never force or hide medications. Staff who assist with medications must complete state-required medication training. Staff may not give injections unless authorized by law; injections are self-administered or given by an appropriately licensed health professional, such as a nurse.
