This policy explains what information VitalPath Health Enterprise, LLC dba Sérénity Senior Navigation and Care Academy (“Sérénity,” “we” or “us”) collects through this website and its forms, how we use and share it, and the choices you have.

Information we collect

We collect the information you choose to give us, plus the basic technical information every website receives.

• Contact details, such as your name, email address, phone number, organization and role, when you fill in one of our forms, ask for an urgent callback, book a consultation or email us.
• Information about a person who needs care, which families and hospital discharge planners send us through our secure, HIPAA-compliant intake forms. This can include health information and medical records.
• Information about care homes in our partner network, such as the facility’s name, license and insurance details, administrator contact details, bed availability, and photos or virtual tours of the home.
• Care Academy application details, such as your contact details, your answers to the application and video interview, documents you upload (for example, proof of Live Scan fingerprinting) and your training progress.
• Account and order details if you create an account or buy a course: your name, email address, billing address and order history. Card payments are handled by our payment processor; we never see or store your full card number.
• Messages you type into the chat assistant. To write a reply, the conversation is sent to an artificial-intelligence service that works for us. We do not save chat conversations, and the conversation is cleared when you leave the page.
• Technical information that browsers send to every website, such as your IP address, browser type and the pages you visit. Our web host may record this in server logs for security and troubleshooting.

How we use information

• To find and arrange care: matching a senior with suitable licensed care homes in our partner network, preparing admission paperwork and coordinating the move.
• To respond to you, including consultation bookings, questions and urgent callback requests. When you ask for a callback, we text your name and phone number to our on-call coordinator so they can call you back.
• To run the Care Academy: reviewing applications, providing training, recording progress and issuing credentials that employers can verify.
• To manage our care home partner network, including bed availability updates and license and insurance renewal reminders.
• To send the service text messages described in our SMS Terms and Conditions, and service emails.
• To process payments for paid courses.
• To keep this website and our systems secure, and to meet our legal obligations.

We do not sell your personal information, and we do not use it for advertising.

Health information

We treat information about a person’s health as protected health information under the federal Health Insurance Portability and Accountability Act (HIPAA) and as medical information under California’s Confidentiality of Medical Information Act (CMIA), and we handle it as those laws require.

• On this website, we collect health information only through our secure, HIPAA-compliant forms. It goes from those forms straight to our HIPAA-compliant systems and is not stored on this website.
• It is stored and processed only by service providers that have signed a HIPAA business associate agreement with us.
• We never send health information by text message or email.
• We share it only to arrange care (for example, with the licensed care homes being considered for a resident and with the transportation provider arranging the move), or as the law requires or allows.
• Uploaded medical records, and the working copies we make from them, are deleted 60 days after the resident moves in. The care home that admits the resident keeps its own records.

Please don’t include medical details in the chat assistant, a callback request or an ordinary email. Use our secure intake forms instead.

Text messages

If you give us your mobile number and agree to receive texts, we use it only to send the messages described in our SMS Terms and Conditions. Reply STOP to any message to stop them.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.

Who we share information with

We share personal information only in these situations:

• With partner care homes and transportation providers, when needed to arrange a placement (see “Health information” above).
• With anyone checking a Care Academy credential. If you graduate, anyone who has your Student ID or certificate QR code can see your name, certification date, completed modules, clinical shadowing verification and Live Scan clearance status on our Verify a Credential page. This lets care homes and state surveyors confirm your training.
• With service providers that run parts of our service for us: website and cloud hosting, secure online forms, our database, email, text messaging, payment processing, maps, and an artificial-intelligence service that writes the chat assistant’s replies and helps us read medical records to prepare admission paperwork. They may use the information only to provide their service to us, and those that handle health information have signed a HIPAA business associate agreement with us.
• When the law requires it, or when needed to protect the safety of residents or others.
• If our business is sold or merged, with the new owner, who must keep protecting the information as this policy describes.

Cookies and similar technologies

This website does not use advertising cookies, social-media pixels or third-party analytics services, and it does not track you across other websites.

• Your browser remembers your accessibility settings (text size and high contrast) and the language you chose, so the site looks right on your next visit. These preferences stay on your device; they are not cookies and are not sent to us.
• If you log in as a care home partner or Care Academy student, WordPress uses cookies to keep you signed in.
• Our online course store uses cookies to run the shopping cart and checkout, and our payment processor may use cookies on the checkout page to help prevent fraud.
• Some pages show content from other services: our secure forms, the Live Scan location map, virtual tours and videos, the consultation booking calendar and the video interview. When that content loads, the service that provides it receives your IP address and may set its own cookies, under its own privacy policy. YouTube videos are shown in YouTube’s privacy-enhanced mode.

Because we do not track you across other websites, the site works the same whether or not your browser sends a “Do Not Track” signal.

How we protect and keep information

This website and our forms use encryption (HTTPS). We limit access to personal information to the people who need it for their work, and we keep health information only in HIPAA-compliant systems. No system is perfectly secure, but we work to protect your information and will notify you of a breach as the law requires.

We keep personal information only as long as we need it for the purposes in this policy or as the law requires. Uploaded medical records are deleted as described under “Health information.”

Your choices and rights

• Text messages: reply STOP to any message to stop receiving texts.
• Access, correction and deletion: you can ask to see the personal information we hold about you, to correct it or to delete it. We may need to confirm your identity first, and we may have to keep some records when the law requires it, such as records connected to a placement or a credential. If we cannot fully meet a request, we will tell you why.
• Health information: you also have the rights HIPAA and the CMIA give you, such as the right to see and get a copy of your health information.

Children

Our services are for adults. We do not knowingly collect personal information online from children under 13.

Changes to this policy

If we change this policy, we will post the new version on this page and update the effective date at the top.
