SMS Terms and Conditions<\/h1>\n

Program name: S\u00e9r\u00e9nity Senior Navigation alerts<\/p>\n

\n These terms apply to the text messages that VitalPath Health Enterprise, LLC dba S\u00e9r\u00e9nity Senior Navigation and Care Academy (\u201cS\u00e9r\u00e9nity\u201d) sends about its services. <\/p>\n\n

Who receives our messages, and what they say<\/h2>\n

Care home administrators in our partner network<\/h3>\n

Bed-availability check-ins about twice a week, which you can answer by text; links to review a placement request; check-ins after a resident moves in; and reminders when your facility license or liability insurance is due for renewal.<\/p>\n

Hospital discharge planners and families<\/h3>\n

Confirmation that we received your placement request, and follow-up messages about that request, such as a check-in after the move.<\/p>\n

Care Academy candidates<\/h3>\n

Updates on the status of your application.<\/p>\n

No resident health information is ever sent by text message.<\/strong><\/p>\n\n

How you sign up<\/h2>\n

You join when you give us your mobile number on one of our forms (the care home partner registration form, a hospital discharge or family intake form, or the Care Academy application) and agree to receive text messages from S\u00e9r\u00e9nity.<\/p>\n

Consent to receive text messages is not a condition of purchase or of receiving any of our services.<\/p>\n\n

How often, and what it costs<\/h2>\n

Message frequency varies. For example, care home administrators receive bed-availability check-ins about twice a week; other messages are sent only when there is news about your request, application or facility.<\/p>\n
