Fingerprints are submitted through the hiring care home on CDSS form LIC 9163. All fingerprinting fees are the direct responsibility of the candidate.<\/p>\n        <\/div>\n        <p>When a partner care home offers you a position, it will give you a California Department of Social Services Live Scan form (LIC 9163) or a Guardian link with its facility number.
